Parental Consent & Waiver (For Participants Under 18)

I, the undersigned parent/guardian, give permission for my child to participate in the Her Game Clinic on April 22, 2024 at Costas Papaellinas Court, Nicosia. I acknowledge and agree that:

  • I understand that basketball is a physical sport and that injuries may occur. I will not hold Her Game, its coaches, staff, or venue partners responsible for any injury sustained during the clinic.

  • I confirm that my child is in good health and physically able to participate. If my child has any medical conditions, I have informed the clinic organizers in advance.

  • I grant permission for my child to be photographed or recorded during the clinic for Her Game promotional use unless I notify the organizers otherwise in writing.


Waiver & Release (For Participants 18 & Over)

I, the undersigned participant, acknowledge and agree that:

  • I understand that basketball is a physical sport and that injuries may occur. I will not hold Her Game, its coaches, staff, or venue partners responsible for any injury sustained during the clinic.

  • I confirm that I am in good health and physically able to participate. If I have any medical conditions, I have informed the clinic organizers in advance.

  • I grant permission for my image to be used in promotional materials by Her Game unless I notify the organizers otherwise in writing.
